Christian Fellowship Homeschool Co-op (“Fellowship”) is now accepting applications for the 2012-2013 school year. We look forward to receiving your application.
Applications must be received by the end of February 5, 2012.

The application contains 4 pages and is a Word form.
Open and complete the Word form and save to your computer's hard drive. When completing this form you can move to each blank using the TAB key. The "Check boxes" can be selected by using the space bar or with your mouse. When finished, save your changes and attach this newly created file to an email and send to membership@cfhco-op.com.
All applications must be received via email!
Please seek out a friend or use the Library’s computers if you are having trouble submitting this from your own computer – remember to CC your own email so that you will have a copy of your Application!
